example 1 isn't working

1. my php is php-5.1.2-Win32 but on the combo box the newest version that i can chose is 5.1.1

2. After copy example1 scripts to my Apache2 htdocs folder i've start it form index.html next example.php and when I fill login form with data i.e. child child or father father or mother mother and click submit button the page after load is empty not even <html> tag appers

after login i should see witch user i've login

Actual result:

blank screen with no text and no html tags
